COVID-19 Vaccination Clinics at Saint Mark

Aloha K-8 Saint Mark Parents,

With the recent approval of the Pfizer/BioNTech COVID-19 vaccine for ages 5 to 11, Saint Mark Lutheran School, in partnership with Kaiser Permanente and the Hawaii Department of Health, is excited to be hosting a vaccination clinic on Friday, November 19 from 9:00 a.m. to 2 p.m. on the church lanai.  An informational flyer is going home with the students today.

Who Can Participate?
The clinic is primarily intended for those aged 5 to 11, but Kaiser Permanente is happy to administer the vaccine (1st dose, 2nd dose or booster) to other eligible family members (older siblings, parents, etc.) who would like to participate.  This clinic is not open to the general public.

Parents Invited to Attend (but not required)
Parents of younger students are invited/encouraged to accompany their child(ren) while receiving the vaccine; however, it is not required so long as all of the proper paper work has been completed ahead of time. The cones will be moved to allow parents to park.

Appointments Highly Recommended (but not required)
It is highly recommended that all participants schedule an appointment; however, appointments are not required as long as supplies last.  CLICK HERE to schedule an appointment.

Required Paperwork
The following forms/documents are required for all those participating in the clinic.  We are asking that the completed forms for all participating students be turned into the school office ahead of time.  If that is not possible, then the completed form must be brought to the appointment.  Any adult participants should bring their forms on the day of the appointment.

2nd Dose on December 10
After participants receive their first dose on November 19, they will be automatically scheduled to receive their second dose during the same time slot on Friday, December 10.
